So You’ve Had a Stroke — Now What?

You have had a stroke. Hopefully, you went to the hospital when you developed your symptoms of weakness, numbness, altered speech or visual impairment. Your hospital care enabled you to limit the damaging effects of the loss of circulation to a portion of your brain. You’ve made it through the acute phase of stroke management. Now what?

You will want to obtain the best achievable outcome from the impairments you already have. If you have “motor” impairments (weakness or clumsiness) you can rest assured that randomized, controlled trials — the gold-standard method for determining a treatment’s effectiveness — have shown that physical therapy can improve your level of functioning. If you have speech impairment, then speech therapy might be beneficial, though this has never been proved by means of randomized, controlled trials.

While it is important to focus on rehabilitation following a stroke, there are also other issues to attend to. As a survivor of a stroke you are at increased risk for another.

Researchers at the Mount Sinai School of Medicine and Columbia University in New York studied 655 people who suffered first ischemic strokes. (Ischemic strokes are due to plugged blood vessels and not bleeds, and comprise 85-90% of all strokes.) Publishing their results in a March 2006 issue of the journal “Neurology,” the investigators found that in the first five years following the stroke there was an 18% likelihood of another. Over the same time period the research subjects also had a 5% likelihood of a heart attack.

Can you improve your odds? Absolutely! The process of using information from the first stroke to help prevent another is called “secondary stroke prevention.” The idea is that if there is something that can and should be done to reduce one’s risk, now is the time to do it. There is no point in waiting for yet another attack to occur before getting started.

A blue-ribbon panel from the American Stroke Association and American Heart Association reviewed the state of knowledge concerning secondary stroke prevention for patients with ischemic strokes and published their results in a March 2006 issue of the journal “Circulation.” They found that use of blood-pressure-lowering medications has a powerful effect in reducing the risk of a second stroke — ranging from 24-43% in better studies — and this benefit might even extend to patients who have normal blood pressure to start with.

If you have diabetes, then it is especially important to control high blood pressure. Using a medication from the groups of drugs known as “angiotensin converting enzyme inhibitors” (ACEIs) and “angiotensin receptor blockers” (ARBs) will not only help control blood pressure, but will additionally help protect the kidneys. If you have diabetes, then it is also important to consider use of cholesterol-lowering medication, especially from the class of drugs known as “statins.” Statins can additionally benefit people without diabetes and even those without elevated cholesterol levels. Of course, in diabetes it is also important to keep the blood-sugar levels as close to normal as is humanly possible.

Quitting smoking is also pivotal in preventing another stroke, and it is never too late in the game to benefit from this difficult but important change. Consumption of more than two standard drinks of alcohol per day also increases the risk of stroke and should be avoided. If you are obese, then it is in your best interests to lose weight through a combination of calorie reduction and sensible exercise.

The carotid arteries are a pair of pulsating blood vessels in the front of the neck that carry blood to much of the brain. If your hospital studies showed that a carotid artery is 70-99% narrowed (severe stenosis) and your recent stroke was downstream from this blood vessel, then you are much less likely to have another stroke if you have a surgical clean-out (endarterectomy) by an experienced surgeon whose complication rate is less than 6%. If you have severe narrowing, but because of some medical or surgical problem the surgery is considered too risky, then insertion of a stent into the narrowed artery can serve as a substitute for endarterectomy.

If the carotid artery on the same side of the stroke is 50-69% narrowed (moderate stenosis), then an endarterectomy can be considered, but the benefit of surgery in these circumstances is much less clear-cut. If the narrowing is less than 50% (mild stenosis), then you are better off leaving the artery alone.

The above recommendations are based on studies in people with atherosclerosis (hardening of the arteries) which is the most common cause of strokes and, for that matter, heart attacks. But not every stroke is caused by atherosclerosis. That’s why medical testing is important in stroke patients — so that treatment can be tailored to individual circumstances.

What about blood-thinners? If you have atrial fibrillation (a specific pattern of irregular heartbeats) then you are at particular risk to have a stroke due to a blood clot being thrown into the circulation from the heart. In this case warfarin (Coumadin) is the blood-thinner of choice. If for some reason the warfarin cannot be tolerated or is considered too risky, then aspirin is a second-best choice.

If your stroke was due to atherosclerosis, then studies support the use of an “anti-platelet” drug. Platelets are the building blocks from which blood clots are made, and anti-platelet drugs interfere with the ability of the platelets to clump together to form a clot. Antiplatelet drugs of first choice include aspirin by itself, aspirin in combination with extended-release dipyridamole (Aggrenox), and clopidogrel (Plavix).

It’s important to realize that patients who address every risk factor for a second stroke are likely to have the best outcome and the lowest chances of another attack. Handling some risk factors and not others is better than doing nothing at all, but in fighting off a second stroke, you want to use every weapon in your arsenal.

listed in the Yellow Book and the other phone

listings in the yellow pages. Provided, you have a
business line, in the name of your business, at
the phone company. Advertising your home number as
a “business” can only lead to confusion and a
problem when the phone company finds out.It’s as
bad for your company image as having an eMail
account with a free service. Spend the bucks for a
real business number and a real Internet domain.

Once you have an “official” business number, the
trick is to get people to remember your name when
they look in the book. That’s advertising’s job.
If you stress what’s in it for them they will
remember who you are when they go looking. Many
times they go looking a year, or more, after
hearing or seeing your ads. If it is all about
them, they will remember all about you.

Surveys show the majority of people look in the
Yellow Pages for a name that seems familiar,
either through past dealings or because of

Yoga and Mantra Meditation

Westerners tend to associate the practice of yoga with the physical regimen of poses, stretches and breathing, commonly known as hatha or astanga yoga. There is much more than this in the ancient art of yoga. Meditation, specifically mantra meditation, is an integral part of the yogic tradition as it helps bring the practitioner closer to the true and ultimate goal of yoga: union with the Supreme.

The Sanskrit word “yoga” means “union” or to “link up with”, specifically referring to linking up with God or the Supreme Being. The practice of hatha and astanga yoga is a preliminary process designed to assist the yogi in controlling their senses to able to focus better on the Supreme. Ultimately the practitioner is able to control and raise their life air through the chakras to attain union.

A mantra is a sound vibration or specific series of words or sounds. The word “mantra” is derived from the Sanskrit word “man” which means “mind” and “tra” which means “to deliver from” or “away”. A mantra is therefore a sound vibration that takes the mind away from the material and puts it on the spiritual. Hence, mantra meditation is often referred to as “transcendental sound” because the sound of the mantra transcends the material world.

Mantras are always derived from names of God or descriptions of attributes of the Supreme Being. Thus, mantras often include Sanskrit names for God such as Rama, Chandra, Krishna and Siva. Also, Sanskrit terms such as ‘Hare” (referring to the energy of God) and “Shanti” (peace) are quite common.

The process of mantra meditation is quite simple. The yogi focuses his attention on the sound of the mantra and repeats it softly to him or herself, or sings and chants it aloud with others. Whenever the mind wanders, attention is brought back to the sound of the mantra. This can be done while using meditation beads (japa yoga), or in groups with musical instruments (kirtan). In time, with practice, the practitioner is better able to focus the mind on the transcendental sound and the mind has less of a tendency to wander.

The purpose of mantra meditation involves a fascinating philosophy. First of all, it is believed that the names of God have a purifying effect on the consciousness. The body is seen as a temporary, material container, housing the spirit soul (one’s true essence) and surrounded by a subtle material body, or the mind. The regular hearing and chanting of mantras is seen to help clear away the confusion and misery caused by identifying oneself with one’s body and mind.

This false bodily identification is seen as the root of all pain and bewilderment. Imagine the spirit soul as a brilliant, shining gemstone but one cannot perceive this because it is covered by dirt and dust. Mantras serve as a sort of transcendental polishing cloth, slowly taking away the dust so that the yogi can begin to experience the true essence of the self - eternal, full of knowledge and bliss!

Mantra meditation is therefore an important aspect of the process of self-realization. This is the ultimate goal of yoga: the realization that we are spirit in essence, part and parcel of the Supreme Spirit. The more we progress on the path of yoga, the more peaceful, blissful and loving we become. This is spiritual life.

Paper Can Become Elegant For Christmas

Making an elegant centerpiece for your table, that you can paint
a design on to match your Christmas table settings.
Here I will show you how to make a elegant bowl to hold fruit, nuts
or sweets.

This will also help recycle your old news papers.

The art of papier-mché is one of the cheapest and easiest crafts
to do and can result in some stunning beautiful pieces.
This can be fun and rewarding to see your own work as center of
attraction.

There are two techniques of papier-mché. Pulping involves mixing
pieces of paper with water and boiling them to a pulp and adding
an adhesive this makes a clay like substance.
This type is used for sculpting and is for the more advanced person.

The second type and most popular is layering, in which layers of
torn news paper are pasted over each other onto a mould, this method
is used for more delicate projects and is the type I recommend for
beginners, and this is the one I will be sharing with you today for
your elegant centerpiece.

The materials you will need are.

News paper
 Several sheets of  waste plain paper
 Flat bottom metal, china or plastic bowl for mould
 Petroleum jelly
 PVA glue
 Scissors
 Craft knife
 Thin cardboard
 Masking tape
 White matt emulsion paint
 Paint brush
 Soft pencil and eraser
 Colored gouache paints
 Non-toxic watercolor varnish

What to do.

1.Tear news paper and plain paper into strips about 10cm to 15cm
long and 2cm to 4cm wide.

Smear a thin layer of petroleum jelly over the mould.
Mix the PVA glue with a little water.

Dip strips of paper into glue mix and start pasting to mould.
To ensure a consistent thickness, alternate layers of news paper
with layers of plain paper.

Apply eight to ten layers of paper, leaving each one to dry before
applying the next.

2.Trim around the top and bottom edges of the bowl with scissors.
Gently insert a knife between the mould and the papier-mché shell,
and separate the two.

Bind the top edge of bowl with more strips of glued paper.
Cut a circle of cardboard to fit the base and secure this to the
outside with masking tape.

Remove the bowl completely from the mould and cover the inside and
outside with two more layers of glued paper strips.

Allow to dry completely.

Note: Apply glued strips at right angles on the last layer to keep
the surface as smooth as possible.

3. Paint the bowl with two coats of white matt emulsion paint to
give a neat strong surface leave to dry.

4. With a pencil lightly draw your design on the surface, then fill
in with gouache paints and again leave to dry. When completely dry
cover with two layers of clear varnish.

I painted mine in a deep red and painted gold stars just here and there for a Christmas theme and filled with assorted sweets hanging candy canes around the edge.

This technique can be applied to most moulds that have straight sides
or even plates and trays.

Plates can make a very attractive wall hanging with a photo of your
family or pets in the center.

Let your imagination go wild and most of all have fun, these projects
are suitable also for supervised children, you can make wonderful
piatas for parties using balloons as moulds and when dry pop the

tape stick the piece you cut out back and decorate with shredded
tissue paper.
